During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/5/2025. The on-site inspection began at 11:05 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 19. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/15/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-50-A
Providers must keep a written record of children in attendance each day. There were 8 children present but only 6 on the list.
Plan of Correction: A note was placed on sign in sheet to sign child name and not parent name.
Standard 8VAC20-800-500-C
Stationary outdoor playground equipment shall be placed at least six feet from the perimeter of other play structures or obstacles. Wooden border was 4.5 feet from one of the sides.
Plan of Correction: More mulch will be bought to be put around perimeter and the barriers will be removed.
Standard 8VAC20-800-690-D
The diapering surface shall be nonabsorbent. The diapering surface had a tear in the pad leaving exposed foam.
Plan of Correction: Replace pad with new pad
Standard 8VAC20-800-830-C
Documentation shall be maintained of emergency evacuation drills. Dates of drills were not documented.
Plan of Correction: The day of the drill not just the month will be added to the documentation on drill day
Standard 8VAC20-800-960-C
High chairs or feeding tables with safety waist and crotch straps fastened according to the manufacturer's instructions shall be used for children under 12 months of age. Child #2, eleven months old, was not strapped in the feeding chair while eating lunch.
Plan of Correction: Child was restrapped correctly with straps and tray.
