Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 3/26/21/21 and concluded on 4/16/21. A self-reported incident was received by the department on 3/25/21 regarding an unattended and unsupervised child. The director was contacted by telephone to conduct the investigation. The inspector did a virtual tour of the center and conducted interviews of staff. The evidence gathered during the investigation resulted in violations of the standards that are listed on the violation report.
Violations
2Evidence:
1. On March 25, 2021, at approximately 4:30pm, child #1 (3 years old) was found alone and unsupervised on the playground by staff #3 after being told that staff #4 could hear him on the playground.
2. According to interviews, child #1 was on the playground with staff #1 and staff #2 in a group of 15 children, and did not come in with the class when they exited the playground.
3. Staff #6 (Program Director) confirmed that child #1 was not always within actual sight and sound supervision of staff.
Evidence:
1. The facility's policy states that employees ?should maintain a position that allows them to monitor all areas of the playground and should do name to face checks to ensure all children are present.?
2. On March 25, 2021, child #1 was found on the playground alone and unsupervised by staff #3.
3. After exiting the playground and returning to their classroom, both staff #1 and staff #2 were unaware that child #1 was left on the playground until they were informed by staff #7.
4. Staff #1 (Program Director) confirmed that child #1 was being supervised by staff #1 and staff #2, and that they did not comply with their own policy for being accountable to the children in care are at all times.
