Inspector Notes

An unannounced renewal inspection was conducted from 10:00 AM - 12:00 PM. At the time of the inspection, there 127 children (ages infant through preschool in care with 20 direct care staff plus support personnel. Children were observed participating in circle time, individual choice activities, art projects, and playing outdoors as well as infant care and medication administration. The sample size was ten (10) staff records and six children's records.

Based on review of six children's records, it was determined that the facility did not ensure that all required elements are present in each child's record.

Evidence:
The record for child #6 contained only one of the two required emergency contact persons.
Plan of Correction: Parent will be contacted to obtain an additional emergency contact person. Records will continue to be audited on an ongoing basis to ensure that they are complete.
