Inspector Notes

A self report investigation regarding allegations in the area of Programs, in addition to a monitoring inspection was conducted on 10/13/2023.There were 124 children present and 27 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 6 children?s records and 8 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-410-1
(Complaint Related) Based on interviews and video footage, the center did not ensure children were not rough handled by staff.
Evidence:
1. On 09/29/2023 at approximately 4:30pm, child #1 was on hiss knees in the block area when Staff #1, standing about 1.5 - 2 feet away, roughly grabbed him above his left elbow and pulled him toward her.

2. Still holding onto the child's arm, staff #1 leaned toward the child's face and with a loud, harsh tone said three times, "What did I tell you to do?"

3. In an interview Staff #1confirmed she used "physical redirection" and an "extremely harsh tone and yelled," at child #1.
Plan of Correction: The staff member has been counseled, placed on probation and has begun mandated training. Management is also monitoring her classroom management with frequent checks over the next 6 months.

Based on a record review the center did not ensure all required information was kept for each child.
Evidence:
Child #1 did not have documentation of first day of attendance. In addition, the director confirmed the child is no longer enrolled and there was not documentation in the record of the child's last day of attendance.
Plan of Correction: The child is no longer enrolled and the first and last day of attendance will be added to the record and retained for the required time frames.
