Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 9, 2024 and completed on September 9, 2024. The on-site inspection began at 10:50am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 97 children present and 21 staff. The inspector reviewed 10 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 1, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.

The screening for staff #1 was completed four months before hire.
Plan of Correction: The staff member will get a new TB test completed.
Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

The screening for staff #2 was expired for eight months after initial TB screening.
Plan of Correction: The staff member had a current TB test and it has been placed in their file.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: when applicable, work phone number and place of employment of each parent who has custody.

1. The record for Child #3 was missing the employment phone number for one parent. The child was present during the inspection and had been in care for 9 months.
2. The record for Child 4 was missing the employment information and employment phone number for one parent. The child was present during the inspection and had been in care for 3 years.
3. The record for Child #1 (in care for 9 months), Child #2 (in care for 2 years), Child #5 (in care for 3 years) and Child #6 (in care for 4 years) were missing the employment information and employment phone number for both parents. The children were present during the inspection.
Plan of Correction: The center has reached out to the families to get any missing work information have updated their files.
