Inspector Notes

An unannounced, on-site renewal inspection was initiated on February 18, 2025 and completed on February 24, 2025. The on-site inspection began at 1:50pm on 2/18/25 and ended at 3:46pm on 2/24/25. The inspector reviewed compliance in the areas listed above. There were 127 children present and 24 staff. The inspector reviewed 10 children?s records and 18 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (3/18/2025). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. The following physical plant safety issues were observed. The infant room had a wall with peeling paint that was within reach of an infant?s crib.
Plan of Correction: Painting has since been covered.
Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied cots, beds, and rest mats. More than half of the 15 occupied cots in the four- and five-year-old classroom had less than 12 inches of spacing in between them.
Plan of Correction: Staff has made sure to space out cots during nap time.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the time when parents were notified. A sample of 18 injury reports were reviewed and 13 of them did not list the time parents were notified.
Plan of Correction: Files have been reviewed and documentation has been updated.
Standard 8VAC20-780-560-G
When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Three infant?s bottles did not have a date on them.
Plan of Correction: Food items are prechecked upon arrival for proper labels.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the name and phone number of child's physician. The record for Child #1, Child #2 and Child #3 did not have the physician information. All three children were present during the inspection and have been in care for a least a month.
