Submit to fingerprinting

The center failed to ensure that a completed fingerprint search was conducted for Staff #1 prior to working with children. Staff #1 does not have documentation that fingerprint search was conducted. The center is 3 months out of compliance.

At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening .

The center did not ensure that staff #3, 4, and 5 have submitted the results of a repeat screening. Staff #3 is 2 months out of compliance, Staff #4 is 1 year 9 months out of compliance, and Staff #5 is 2 years out of compliance.

Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

The center did not ensure that Staff #1 and Staff #3 completed the required orientation training within 7 days of the date of assuming job responsibilities. Staff #1 is 3 months out of compliance and Staff #3 is 3 years and 8 months out of compliance.
