Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interview.

A monitoring inspection was initiated on 02/25/2022 and concluded on 03/07/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 22 children present, ranging in ages from 5 years to 11 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record reviews, the facility failed to obtain a repeat tuberculosis screening on staff every two years.
Evidence:
1. The inspector reviewed two staff records. Staff record #1 contained a tuberculosis screening dated 02/04/2019.
Plan of Correction: Staff #1 completed a tuberculosis screening today and results will be sent to the licensing office on 03/09/2022.
