Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.

A monitoring inspection was initiated on August 25, 2021 and concluded on August 27, 2021. The director was contacted by telephone and email to initiate the inspection. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of documentation, the center did not provide to the licensing representative an annual fire inspection report that was not more than one year old.

Evidence: The most recent fire inspection report for the facility was more than one year old and was dated 04/21/2020.
Plan of Correction: The center has obtained an updated fire inspection.
