Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/23/2024. The on-site inspection began at 9:45 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due 2 months ago and Staff #2 updated screening was due 3 months ago. The updated screenings have not yet been completed.
Plan of Correction: Screenings have been done
and documentation is in staff
files.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. Child #4 has a diagnosed medication allergy and was not included on the list.
Plan of Correction: We will review hard copy health forms
to ensure any allergies listed there
also appear on the allergy list, which
accomplished by ensuring the
allergies are entered into Renweb portal
by the parent, from which the list is
generated.
Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid. Staff #1 and Staff #2 certifications in CPR and First Aid had expired one month ago. They were supervising a group of 14, 4 and 5 year old's.
Plan of Correction: Training has been completed and
certificates are in files.

(Repeat Violation) Centers must keep a separate record for each child enrolled that contains all of the required information.
Child #1's record was missing the place of employment phone number for one of the parents; the address for one of the emergency contacts; name, address, and phone number for a second emergency contact.
Child #2's record was missing the place of employment and phone number for both parents; the address for one of the emergency contacts; name, address, and phone number for a second emergency contact.
Child #3's record was missing names, addresses, and phone number's for two emergency contacts; name of any additional programs or schools the child is concurrently attending; required written parental agreements.
Child #4's record was missing the address for one of the emergency contacts; name, address, and phone number for a second emergency contact; name of any additional programs or schools the child is concurrently attending.
