Inspector Notes

A monitoring inspection was initiated on 8/9/2022 and concluded on 8/9/2022. There were a total of 87 children with 14 staff members present. Children were observed during snack time, engaged in group activities, puzzles and outdoor play time. A total of 3 child records and 3 staff records were reviewed. The inspector observed the renovations that included 3 new doors which lead to the outside in three different classrooms and 2 new bathrooms in the two's classroom. One additional classroom that is now being used for care was measured. Discussed that the building official must approve the renovations and the director must submit a revised building request form to change the license. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of staff records, staff have not submitted results of fingerprinting prior to first day of employment.
Evidence: The following staff did not submit results of fingerprinting prior to first day of employment;
Staff#1, date of employment 1/24/2022, date of fingerprint results 2/3/2022.
Staff#2, date of employment 1/17/2022, date of fingerprint results 1/28/2022
Plan of Correction: Going forward all staff will obtain fingerprint results prior to hire date.
Standard 8VAC20-780-160-A
Based on a review of staff records, each staff member has not submitted documentation of a negative tuberculosis screening at the time of employment or within the last 30 days of the date of employment.
Evidence: The following staff did not complete TB screening within 30 days of first date of employment; staff #1 date of employment 1/24/2022, date of TB screening 2/4/2022
Staff #2 date of employment 1/17/2022, date of TB screening 9/20/2021.
Plan of Correction: Going forward, all newly hired staff will obtain TB results prior to hire.

Based on a review of center procedures and Director interview, the center has not developed and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
Evidence: The Director stated that there was no written procedure.

Standard 8VAC20-780-40-K
Based on review of center procedures and Director interview, the center has not developed written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence: The Director stated there was no written procedures.
Plan of Correction: A written procedure will be completed and shared with staff.

Based on observation and Director interview, the facility is not operating within the terms of its license, which include the maximum number of children who may be in care at any time.
Evidence: The current license says that currently 83 children are allowed at any one time. During the inspection on 8/9/2022 there were 87 children in care.
Plan of Correction: The director will request to increase the capacity since another classroom within the church is being used for childcare.
Standard 8VAC20-820-120-A-7
Based on observation and Director interview, the facility is not operating within the terms of its license, which include other limitations.
Evidence: There is a limitation on the current license that no more than 13 children under the age of 2 1/2 years may be in care at any one time, by order of the Building Official. During the inspection on 8/9/2022 there were approximately 25 children ages 2 months through 24 months within 2 separate classrooms.
Plan of Correction: Three more rooms now have direct access to the outside allowing no limitation to children under the age of 2 1/2 years. The Director will get a revised Building Inspection approval to remove the limitation from the current license.
