Inspector Notes
An unannounced Monitoring inspection was conducted today. A total of 6 children (14pts) with 2 staff providing supervision was onsite. The children were preparing for lunch time. They were seated at the table while staff were observed serving lunch. Lunch today was chicken with rice and fresh slices of cucumber. They had completed a morning activity which had the children paint and color the letter "N". The artwork was observed drying on bookshelves. After lunch, the children transitioned to playtime. The younger children were placed in cribs for nap time. The older children transitioned smoothly to their designated cots and quickly fell asleep. Observation of the all programming areas was made. Consultation provided regarding the area used for diapering. No medication needs and no food allergies were identified. A total of 4 children's records and all staff records were reviewed. A second emergency contact was not found for Child #3.
The child's start date was not found.
Evidence:
Staff #2 failed to wash their hands after diapering one child and proceeded to diaper another child directly afterwards.
Evidence:
Staff #2 failed to ensure that Child # 4 and Child #3's hands were not washed after diapering.
Evidence:
Staff #2 did not clean with soap and water and sanitize with bleach and water after diapering two children.
Evidence:
Topical ointments were accessible in the crib room and also below the diaper changing surface.

Child # 1 began care on 04/25/23, immunizations were not obtained prior to start date and are not on file.
