During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/26/2025, as a part of the licensure period. The on-site inspection began at 11:15a.m. and ended at 12:45p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 12. The inspector reviewed 5 children?s records and 3 caregivers and 1 household member record on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The most recent Inspection Summary was not posted in a conspicuous place on the licensed premises.
Plan of Correction: Will post this inspection report on the board.
Standard 8VAC20-800-120-B
Assistants' and substitute providers' records shall contain the date of employment.

Staff #1 and #2's record did not contain their date of employment. Staff #1 has been employed for approximately 2 months. Staff #2 has been employed for approximately 1 month.
Plan of Correction: Will obtain start date for both staff and document within their file.
Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

A cleaning agent was stored in an area accessible to children in the bathroom.
Plan of Correction: Removed and placed in an area out of reach of children.
Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.

A written record of children in attendance was not kept. There were a total of 5 children in care during the inspection and there was no record of attendance.
Plan of Correction: Will return to keeping a written record of attendance daily.
Standard 8VAC20-800-60-B
A child's record shall contain the following information: parent work addresses and emergency medical authorization.

Child #1's record was missing work addresses for both parents. Child #1 was present and has been in care for approximately 6 weeks.
Child #3's record was missing the emergency medical care authorization. Child #3 has been in care for 1 day.
Plan of Correction: Will obtain this information from the parents.
