Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on December 13, 2021 and concluded on December 13, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were zero children present and one provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on the review of two children's records, two records did not contain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
The records for Child #1 (first day of attendance 11/17/2020) and Child #2 (first day of attendance 12/28/2020) did not contain documentation of a physical exam by or under the direction of a physician.
Plan of Correction: I will reach out to the families and request documentation of a physical exam. I will send proof to the licensing inspector once a receive it.
