Inspector Notes
An unannounced renewal inspection was conducted on Thursday, February 8, 2024, to determine the provider?s compliance with licensing standards. The inspector was on site from approximately 9:55am until 11:40am. During the inspection, there were a total of 11 children in care, totaling 30 points, in the direct care of three caregivers. The children were observed during free-play, dancing, singing, and watching television. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The caregivers were observed having positive interactions with the children in care. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, medication is not administered and transportation is not provided at this time. Three caregivers' records and five children's records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: The national fingerprint-based criminal record search in the record of Caregiver #3,
employed on 10/10/23, was completed on 10/18/23.
During interview, Caregiver #1 confirmed the fingerprint-based criminal record search for Caregiver #3 was not received prior to employment.
Evidence: The record of Child #4, date of attendance 04/04/23, and the record of Child #5, date of attendance 04/04/23, did not contain documentation of a physical examination.
During interview, Caregiver #1 confirmed Child #4 and Child #5 do not have documentation of a physical examination on file.
Evidence: 1. The record of Child #1, date of attendance 03/17/23, was missing proof of the child's age and identity.
2. The record of Child #3, date of attendance 09/11/23, was missing the parent's signed acknowledgement of the receipt of the information required by 8VAC20-800-70 regarding whether or not there is liability insurance on the family day home operation.
3. The record of Child #4, date of attendance 04/04/23, and the record of Child #5, date of attendance 04/04/23, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.
4. During interview, Caregiver #1 confirmed the required information was missing from the children's records.
Evidence: 1. The immunizations in the record of Child #4, date of attendance 04/04/23, were dated 04/05/23.
2. The immunizations in the record of Child #5, date of attendance 04/04/23, were dated 04/05/23.
3. During interview, Caregiver #1 acknowledged the immunizations for Child #4 and Child #5 were obtained after they attended the family day home.
