Inspector Notes

An unannounced on-site monitoring inspection was initiated on 07/22/2025 and completed on 07/22/2025. The on-site inspection began at 10:50 am and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 237 children present and 27 staff. The inspector reviewed 14 children's records and 9 staff records on-site on 07/22/2025.

Standard 8VAC20-780-240-A
Repeat Violation
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Staff 5 and staff 6, who have been employed for more than 12 months, have not completed the Virginia Department of Education orientation course.
Plan of Correction: The program director stated that both staff are in the process of completing this training.
Standard 8VAC20-780-70
Staff records shall contain all of the required elements.
The record for staff 1, 2, 3, and 4 lacked documentation of two or more references as to character and reputation as well as competency. These staff were working with children during the inspection and all have been employed for more than 30 days.
Plan of Correction: The program director stated that reference checks were now being conducted by HR. The PD will reach out to HR to verify and obtain copy for file.
