Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 12/21/2021 and concluded on 12/21/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 0 children present, with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's record and 2 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

The record of child # 1 (start date: 11/30/21) and child #2 (start date: 7/5/21) did not contain documentation of each child's medical insurance policy number.
Evidence:
1. The record of child #1 (start date: 11/30/21) had documentation of a immunization record that did not disclose if the child was adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
2. The record of child #2 (start date: 7/5/21) had documentation of immunizations dated 7/7/21.
