Standard 8VAC20-800-180-A
Based on record review, the provider did not ensure to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. The record of caregiver #1 had Tuberculosis results dated 8/22/2020, and did not have documentation of subsequent Tuberculosis screening or results.
2. The record of caregiver #2 had Tuberculosis results dated 8/22/2020, and did not have documentation of subsequent Tuberculosis screening or results.
Plan of Correction: I will get it done as soon as possible. I wasn't aware that it should be done every two years.
Standard 8VAC20-800-350-E
Based on observation, the provider did not ensure hot water at taps available to children were maintained within a range of 105?F to 120?F.
Evidence:
The hot water tap located in the provider?s bathroom had a water temperature of 131?F.
Plan of Correction: I am going to call a plumber to come and adjust that.
Standard 8VAC20-800-920
Based on observation and an interview, the provider did not ensure that a menu was given to parents or posted or placed in an area accessible to parents.
Evidence:
1.The family day home did not have a menu posted.
2. The provider stated that they did not have a menu posted or given to parents during the month of December.
Plan of Correction: I will put a menu up.
