During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site Renewal inspection was initiated on 2/11/2025 and completed on 2/19/2025.The inspector reviewed compliance in the areas listed above. There were 32 children present and 4 staff. The inspector reviewed 5 children?s records and4 (all) staff records on-site on 2/11/2025. This inspection included document review, tour of the facility, interviews, observations, measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4.
Plan of Correction: The program director will speak to child #4's parent today to advise that the results of a physical exam must be submitted within 30 days.
Standard 8VAC20-780-150-B
(Repeat Violation) Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization records for child #4 and child #5 were not signed.
Plan of Correction: The program director will contact the parent s of child #4 and child #5 to request a signed copy of their child's immunization record and will submit it to the program within 30 days.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for over five months, was missing the following required components: Address and Phone number for one of the 2 required individuals. The record for child #4 does not contain documentation of their first date of attendance.
Plan of Correction: The program director will contact the parents of child #1 and obtain the address and phone number for the listed emergency contact by tomorrow. The program director will review attendance records to determine and document the first date of attendance for child #4.
