Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 24, 2026. The on-site inspection began at 3:35 pm and ended at 5:15 pm. The inspector reviewed compliance in the areas listed above. There were twenty-four children present and two staff. The inspector reviewed five children?s records and three staff records on-site and electronically on April 1, 2026. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 9, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1 and Child #3, who had been enrolled for six months, did not have documentation of an immunization record on file.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who had been enrolled for six months.

Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child #5 was onsite, but the medication authorization expired six months prior. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.
