Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 3/17/2022. The area manager was contacted by telephone and a virtual inspection was conducted. There were 35 children present, ranging in ages from 5 to 12 years old, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the licensee did not obtain a search of the child abuse and neglect registry or equivalent registry from any state in which any individual staff has resided in the preceding five years.

Evidence:
Three staff records were reviewed.
The record for staff #2 contained documentation of staff #2's residence in New York within the past 5 years.
The record did not contain a Child Abuse / Neglect Registry check from New York.
The record for staff #1 contained documentation of staff #1's residence in North Carolina within the past 5 years.
The record did not contain a Child Abuse / Neglect Registry check from North Carolina.
The district manager confirmed that this documentation had not been obtained.
Plan of Correction: The North Carolina Central registry background check for staff #1 will be requested within 10 days and results filed in the staff record. A copy of the requests will be forwarded to the licensing inspector as will the results, upon receipt.
The request for staff #2's New York Central Registry search has been sent and a copy of the request was sent to the licensing inspector and the results will be forwarded to the inspector upon receipt.
