Inspector Notes

A monitoring inspection was initiated and concluded on 7/26/2023. There were 25 children present with 5 staff supervising. Children were observed having an afternoon snack and engaged in table top and gross motor activities. Hand washing and sanitizing procedures were observed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 11:55 am and concluded at 2:55 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

***REPEAT VIOLATION***
Based on record review and interview, the licensee did not always obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Five staff records were reviewed.
The record for staff #5, who documented having lived in the state of Pennsylvania within the past 5 years, did not contain results of a criminal records search for Pennsylvania.
The record for staff #5, who documented having lived in the state of Pennsylvania within the past 5 years, did not contain results of a child abuse and neglect registry search for Pennsylvania.
Staff #4 verified that this documentation was not available on site and the area manager was unable to provide this documentation following the inspection.
Plan of Correction: The out of state Criminal History record check was obtained with a completion date of 7/27/2023.
The area manager is following up with the PA department regarding the results of the Abuse and Neglect registry results for Pennsylvania and will follow their instruction and/ or submit a new request within 10 days and provide the inspector a copy of the documentation to demonstrate this upon completion.

Based on a review of records and documentation as well as interview, the licensee did not always ensure that the written record of children's injuries include the required signatures.
Evidence:
The program's injury log and accident reports were reviewed.
One injury report contained a single staff signature and did not did not contain two staff signatures or one staff signature and the signature of the injured child's parent.
Staff #4 confirmed that only one signature was documented on the report.
Plan of Correction: Staff #4 stated that she will remind staff and ensure that written accident reports contain two staff signatures or a staff signature and a parent signature effective immediately.
