Inspector Notes

A monitoring inspection was initiated on February 13, 2023 and concluded on February 14, 2023. There were twenty-eight children present with two staff supervising. A third staff arrived during inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 1 of 3 staff have documentation of a negative tuberculosis screening prior to beginning employment.

Based on review and interview, facility did not ensure 2 of 3 staff complete the Virginia Department of Education (VDOE) sponsored orientation course within 90 calendar days of employment.

Evidence: The records for staff #2 (start date 10/14/2022) and staff #3 (start date 3/18/2022) did not have documentation of completing the VDOE sponsored orientation course. Staff confirmed it has not been completed.

Based on observation and interview, facility did not ensure there is at least 1 staff per 18 children when the youngest in the group is ages 5 - 9 years.

Evidence: According to review of head count sheets, on February 13, 2023 at 4:00 PM, 2 staff were present with 39 school age children ranging in age from 5 - 10 years old. Staff confirmed the ratio and stated the third staff member was late arriving to the program. A third staff arrived during inspection.
