Inspector Notes
An unannounced, on-site monitoring inspection was completed on November 14, 2024. The on-site inspection began at 2:50 pm and ended at 4:00 pm. The inspector reviewed compliance in the areas listed above. There were thirty-eight children present and four staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 2, 2024. A POC submitted after this date will not appear on the public website.

Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian. There was no written parent/guardian or physician authorization to administer prescription medication to Child #1. Medication was administered to Child #1 in November 2024.

The center shall maintain a record of the dates of the practice emergency drills for one year. Emergency drill log was not available for prior to July 2024.

Standard 8VAC20-780-60-A
(Repeat violation) Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1 was missing start date and a second emergency contact. The record for Child #2, enrolled for two months was missing one parent home address, one parent place of employment, emergency contact addresses and a physical examination. The records for Child #3 and Child #4 was missing start date.
