Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the facility.
A monitoring inspection was initiated and concluded on 3/17/2022. The area manager was contacted by telephone and a virtual inspection was conducted. There were 25 children present, with 3 staff during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Two staff records were reviewed.
The record for staff #2 did not contain results of a Search of the Central Registry.
The area supervisor confirmed that this documentation was not available.
Evidence:
Two staff records were reviewed.
The records for staff #1 and Staff #2 did not contain documentation of a negative Tuberculosis screening.
Staff #1 was hired 9/15/2020 and staff #2 was hired 6/10/2021.
Staff #1's record had documentation of a Tuberculosis (TB) test being administered, but did not contain the results of the test.
The regional supervisor confirmed that the TB screening results were not in either staff's record.

Two children's records were reviewed.
The record for child #2 did not contain addresses for the 2 emergency contacts listed on enrollment paperwork to be contacted in case of an emergency if he child's parents cannot be reached.
