Inspector Notes
A monitoring inspection was initiated and concluded on 5/8/2024. There were 25 children, ages 4 to 11 years old, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed arriving t the program, washing hands, having an afternoon snack doing staff-led stretching exercises, completing homework assignments and engaged in table top activities and as gross-motor activities. There is no medication being administered to children in care or stored on site for children.
A total of 5 child records and 3 (all) staff records were reviewed. The inspection started at 4:00 pm and concluded at 5:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Three staff records were reviewed.
The record for staff #3, whose date of hire was documented as 9/10/2023, contained documentation that staff #3 resided in the state of California within the previous 5 years.
The record did not contain results of a criminal history record information check or a sex offender registry check for California. Staff confirmed that this documentation was not available for review.
Evidence:
Five children's records were reviewed.
The record for child #5 contained an immunization record which was not signed o stamped by the child's physician, his designee, or an official of a local health department.
Staff will review the immunization records for children at the time of enrollment to ensure they are signed or stamped accordingly.
