During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/30/2025 and completed on 10/7/2025. The inspector reviewed compliance in the areas listed above. There were 52 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records [n-site on 9/30/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-150-B
The required immunizations documentation for enrolled children must include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
The records for child #2 and child #3 contained immunization documentation which were not signed or stamped by the child's physician, his designee, or an official of a local health department.
Plan of Correction: The parents of child #2 and child #3 will be contacted and asked to have their child's immunization documentation signed or stamped by the doctor and submitted tot he program within 2 weeks.
