Inspector Notes

An unannounced monitoring inspection was conducted on March 18, 2024 from approximately 3:35 pm to 4:30 pm. Two staff were present supervising twenty-three school age children. A third staff arrived during inspection. Children were observed arriving to the program, washing hands, eating a afternoon snack and participating in various table activities. Five child and two staff records were reviewed.
Violation was found that is noted on the violation notice.

Based on review, facility did not ensure 1 of 5 child records has documentation of required information.

Evidence: The following was found during record review:
1. The record for child #1 (start date 1/22/2024) did not have a physical examination.
2. The record for child #2 (start date 9/5/2023) did not have a second emergency contact other than parent.
Plan of Correction: Information will be requested from parents and added to child's record.
