Inspector Notes

A monitoring inspection was conducted on 5/10/2022 with center director and center staff. There were 76 children present, ranging in ages from 3 years to 12 years with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed. *Additional documented information was received May 13, May 17, and May 24, 2022. The children were engaged in a variety of activities including arriving from school, doing homework, eating dinner, and playing outside.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-350-B-4
Based on observation, the center has not ensured staff-to-children ratios are maintained for children age 3 years to school age eligibility.
Evidence: A ratio waiver request was granted on 12/23/2021 increasing the ratio in the preschool room from 1 staff to 10 children to 1 staff to 11 children. On 5/10/2022, there were 12 children observed in each preschool room with 1 staff. At approximately 4:00 p.m., one staff returned from the bus run and brought one preschool room into the appropriate ratio.
Plan of Correction: Owner stated there was confusion about the ratio waiver. The correct ratio will be maintained for all ages.
Standard 8VAC20-780-60-A
Based on staff interview, the facility failed to have a record for each enrolled child kept at the center as required. Evidence: the director stated that some of the children's files were currently at the other center to be updated.
Plan of Correction: The files for all enrolled children will be maintained at the site.
