Inspector Notes

A renewal inspection was conducted on 1/24/2023 with the center administrator and staff.
There were 82 children present, ranging in ages from 3 years to 12 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 1 staff records were reviewed. The children were arriving from school, eating dinner, doing homework, playing outside, and having free play with age appropriate toys.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-60-A
Based on review of 8 children's files, the facility failed to have all required information in each child's file. Evidence: Child 1 (date of enrollment 8/29/2022) and Child 2 (date of enrollment 8/29/2022) did not have a signed agreement between the parent and the center in each child's file by the first day of attendance that includes:
1. An authorization for emergency medical care should an emergency occur,
2. A statement that the center will notify the parent when the child becomes ill
and that the parent will arrange to have the child picked up as soon as possible, and
3. A statement that the parent will inform the center within 24 hours or the next
business day after his child or any member of the immediate household has
developed any reportable communicable disease.
Plan of Correction: The agreements were signed by the parents.
