Inspector Notes

An unannounced inspection was initiated on September 25, 2024 and completed on October 16, 2024 with a local agency, in response to a complaint received by the licensing office on August 30, 2024 relating to allegations of supervision. The inspector reviewed 1 staff record on-site. This inspection included document review and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. On August 27, 2024, Staff 1 did not check the vehicle to make sure all children had exited. Child 1 fell asleep on the bus and woke up in the driver's driveway. Staff 1 came out when Child 1 was heard crying. The child was alone on the bus from approximately 8:30 a.m. to 8:35 a.m.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff. On August 27, 2024, Staff 1 transported children from the center to local schools. Staff 1 did not check the bus to ensure all children were off the bus. Staff 1 did not realize that the child was still on the bus until he heard the child crying. The child was alone on the vehicle from approximately 8:30 a.m. to 8:35 a.m.

Standard 8VAC20-780-40-E
The licensee shall ensure that the center's own policies and procedures that are required by these standards are maintained in compliance. The center's transportation policy is for the driver of the bus to "thoroughly check the vehicle for any children or items each time you leave the vehicle". Staff 1 failed to check all seats upon exiting the vehicle on 8/27/2024. The center's incident reporting policy states "You must report to management and document any and all incidents as soon as they occur." Staff 1 did not report the incident to management.

Standard 8VAC20-780-580-K
Staff shall verify that all children have been removed from the vehicle at the conclusion of any trip. On August 27, 2024, Staff #1 did not check that the center's vehicle was empty after morning school drop off. Child 1 (age 6 years) was left on the bus asleep. Child 1 was unsupervised from approximately 8:30 a.m. and 8:35 a.m. on August 27, 2024.
