During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 3, 2025 and completed on September 8, 2025. The on-site inspection began at 10:00 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 18. The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records on-site on September 3, 2025.

This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 15, 2025. A POC submitted after this date will not appear on the public website.

Providers must obtain a initial tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The form shall have been completed within the last 30 days and be signed by a physician, physican's designee, or an official of local health department.
Staff 2, who has been caring for children for a month, initial tuberculosis screening was completed 12 months ago.
Plan of Correction: Assistant went and got tested. Sending results 9/9/2025
