Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 01/05/2022 and concluded on 01/12/2022. The director was contacted by email to initiate the inspection. There were 25 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.036-A
Based on review of two staff's records, the center failed to ensure that all out of state required backgrounds were obtained.

Evidence:
1. Staff # 2 lived out side of Virginia in the past five years and did not have documentation of the out of state sex offender required.
Plan of Correction: The director will obtain the missing background check as soon as possible.
Standard 8VAC20-780-440-G
Based on observation and discussion with the director, the center failed to ensure that all children 12 months and younger were using a crib.

Evidence:
1. On the day of the inspection there were four children 12 month and younger that were sleeping on a infant bed. This is a Montessori approach for the infants to sleep on this type of bed.
Plan of Correction: The director plans on submitting an AV for the use of infant bed that are apart of the Montessori approach.
