Inspector Notes

An unannounced, on-site renewal inspection was conducted on 2/26/25 from 11:00 am to 12:00pm. The inspector reviewed compliance in the areas listed above.There were 9 children present and 3 caregivers; the point total was 21. The inspector reviewed 3 children?s records and 3 caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Caregiver # 1 did not have any training hours completed during 2024.
Caregiver # 2 completed only five out of sixteen clock hours of annual training.
Plan of Correction: Caregivers will complete the required trainings and document.
