A monitoring inspection was conducted on March 30, 2022 concerning a self reported staff supervision incident. Interviews were conducted and electronic data was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview with management, review of electronic information a child under 10 years of age was not always within actual sight and sound supervision of staff.
