During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site renewal inspection was conducted on 3/5/25. The on-site inspection began at 10:30am and ended at 2:38pm. The inspector reviewed compliance in the areas listed above. There were 81 children present and 16 staff. The inspector reviewed 9 children?s records and 11 staff records while on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/18/25. A POC submitted after this date will not appear on the public website.
Violations
5An out-of-state criminal record check shall be obtained within the first 30-days of employment.
Staff #5 began working in June 2024, the out-of-state criminal record check was not requested and completed until October 2024.
There was no documentation of immunizations on-file for Child #14 by the time attendance began in February 2024. Documentation of immunization status was dated January 2025.
Children's hands shall we washed with soap and running water after toileting/diapering.
Children's hands were not washed with soap and water after diapering in 2 classrooms today.
There was not a working battery-operated radio in the school.
1 written injury report did not document how the parent was notified of the child's injury, and another report did not document future actions to prevent recurrence. A total of 5 written injury reports were reviewed.
