Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.

A subsidy health and safety inspection (SHSI) was initiated on August 4, 2021 and concluded on August 4, 2021. There were three children present with one provider supervising. A monitoring inspection was also conducted on this date.

Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-150-C
Based on record review and interview, the vendor did not ensure one household member undergo a background check in accordance with ? 63.2-1725 of the Code of Virginia; (ii) shall not have been convicted of a barrier crime as defined in ? 19.2-392.02 of the Code of Virginia ; and (iii) is not the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

Evidence:
The preliminary information gathered for the remote inspection indicated Household Member #1 was residing in the home. The provider stated Household Member #1 moved into the home in March 2021. The vendor stated a fingerprint based background check, findings of a central registry search and a sworn statement had not been obtained for Household Member #1 as of 08/04/2021.
Plan of Correction: I will have the Household Member complete all required background checks this week.
Standard 22VAC40-665-210-A
Based on the review of four children's records, the vendor did not ensure documentation of immunizations were obtained prior before two children began attending the family day home.
Evidence:
The record for Child #1 (first date of attendance 02/10/2021) contained documentation of immunizations dated 02/22/2021. The record for Child #2 (first date of attendance 04/13/2021) contained documentation of immunizations dated 07/07/2021.
Plan of Correction: I will not let the children attend until all required paperwork has been submitted.
Standard 22VAC40-665-410-A-1
Based on record review and interview, the vendor did not ensure evacuation procedures were practiced at least monthly.
Evidence:
The record of emergency drills conducted in 2021 did not indicate an evacuation drill was completed in March 2021. The vendor stated an evacuation drill was not completed in March 2021.
Plan of Correction: I accidently forgot to complete a fire drill in March 2021. I will make sure all required drills completed within the correct timeframe.
