Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.
A subsidy health and safety inspection (SHSI) was initiated on August 4, 2021 and concluded on August 4, 2021. There were three children present with one provider supervising. A monitoring inspection was also conducted on this date.
Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the program.

The preliminary information gathered for the remote inspection indicated Household Member #1 was residing in the home. The provider stated Household Member #1 moved into the home in March 2021. The vendor stated a fingerprint based background check, findings of a central registry search and a sworn statement had not been obtained for Household Member #1 as of 08/04/2021.
Evidence:
The record for Child #1 (first date of attendance 02/10/2021) contained documentation of immunizations dated 02/22/2021. The record for Child #2 (first date of attendance 04/13/2021) contained documentation of immunizations dated 07/07/2021.
Evidence:
The record of emergency drills conducted in 2021 did not indicate an evacuation drill was completed in March 2021. The vendor stated an evacuation drill was not completed in March 2021.
