Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews.
A monitoring inspection was initiated on 08/13/21 and concluded on 08/13/21. The director was contacted by phone and virtual inspection was conducted. There were 15 school age children present, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the center did not ensure care, protection, and guidance for a child.
Evidence:
1. Staff #1 stated that medication for child #1 was at the main site and not at the site where the child receives services.
Plan of Correction: The child recently was withdrawn from the program. The center will make sure any medications are transferred to the site where the child is enrolled.
