A self reported inspection occurred on 10/25/23 and was completed on 10/31/23. Areas of staffing and supervision were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
Based on a self report and review of documentation the center failed to ensure the care, protection, and guidance of a child on 10/25/2023 as required.

Evidence:
A two year old child was left unattended outside on the fenced in playground on 10/25/23 at approximately 11:09am.
Staff gathered the group of children to head inside to the classroom at approximately 11:09am. Staff did not know the child did not come in with the group until a head count was completed at approximately 11:10am in the classroom. Staff immediately went back outside and found the child playing.
The child was unattended for approximately 2 minutes.
The staff were not able to ensure the child's care, protection, and guidance during this period of time.
Plan of Correction: Staff were spoken to about proper procedures for accounting for children before and after going outside. Staff were also reminded of safety procedures and expectations.

Based on a self report and review of documentation the center failed to ensure children under 10 years of age are always in actual sight and sound supervision as required.

Evidence:
On 10/25/23 at approximately 11:09am the staff gathered the group of children together and headed inside to the classroom from the playground. When the head count was done at approximately 11:10am a 2 year old child was unaccounted for.
This 2 year old child was unsupervised and alone on the fenced in playground for approximately 2 minutes.
Plan of Correction: Staff were spoken to about proper procedures for accounting for children before and after going outside. Staff were also reminded of safety procedures and expectations.
