An unannounced monitoring inspection was initiated on 4/8/2025 and completed on 4/8/2025 in response to a self-report received by the licensing office on 4/8/2025 relating to Staff Qualifications and Training and Staffing and Supervision. The inspector reviewed two children?s records and three staff records electronically on 4/10/2025. This inspection included interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director.
While one or more children were present, there was not a staff member who met the qualifications of a program leader or program director for 34 minutes.
