Inspector Notes

An unannounced monitoring inspection was conducted on 05/16/2023 from 11:40 a.m. to 2:00 p.m. There were 58 children present, ranging in ages from three to five years old with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Based on review of records, the center failed to ensure that the Virginia Department of Education orientation course was completed within 90 calendar days of employment.
1. Staff #3?s start date on file was 08/22/2022. There was no evidence that Staff #3 had completed the VDOE orientation course.
2. Staff #4?s start date on file was 08/22/2022. There was no evidence that Staff #4 had completed the VDOE orientation course.
3. Staff #5?s start date on file was 01/30/2023. There was no evidence that Staff #5 had completed the VDOE orientation course.
4. Staff #6?s start date on file was 10/17/2022. There was no evidence that Staff #6 had completed the VDOE orientation course.
5. Staff #7?s start date on file was 05/16/2023. There was no evidence that Staff #7 had completed the VDOE orientation course.
Plan of Correction: We have added the evidence of completion of the VDOE Orientation Course is on the checklist /for new hires/returning employees requirements. All documents have been sent to VDOE licensing agent, Amy Tomblin, in a separate email on 5/18/23.
Standard 8VAC20-780-330-B
Based on observation and interview, the center failed to ensure that were playground equipment is provided, resilient surfacing complies with minimum safety standards.
Evidence:
1. In the front play area, one climber was observed to be on grass with no resilient surfacing.
2. Staff #1 confirmed that the climber did not have resilient surfacing.
Plan of Correction: Mulch will be placed around climbing apparatus in the front playground. It will meet the standard requirement of 6 inches. This will be completed over the summer and before any children return to school in the fall.
