Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
Neither classroom had a list of allergies, sensitivities and dietary restrictions.
Plan of Correction: A current list of all known allergies sensitivities, and dietary restrictions was created. Each list is posted in a classroom cabinet and will be updated as needed.

REPEAT VIOLATION
The medication authorization shall be available to staff during the entire time it is effective.
In Classroom 2, one child had emergency medication, but the medication authorization was not available to staff.
Plan of Correction: The medication authorization form was placed in the classroom folder. We will
ensure any medical authorization forms will be placed in the classroom folder and updated as needed.

The center shall maintain a written record of children?s serious and minor injuries which includes the required components.
Two injury reports were viewed during the inspection, and they were both missing the following required component: any future action to prevent recurrence of the injury.
Plan of Correction: The two injury reports were completed with the pertinent information. We will ensure the injury reports are completed in it's entirety.
