An unannounced, on-site monitoring inspection was conducted on June 27, 2024 from 2:30 p.m. to 4:30 p.m. in response to a self reported incident regarding medication and supervision. The inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. On June 5, 2025, staff found half of a capsule pill on the floor in the downstairs playroom. Staff observed a preschool child playing with something and then noticed a powdery substance on the child?s shirt. Unsure if the child ingested any of the pill, staff contacted 911 and the child was deemed to be safe. In a written statement, Staff 1 stated "I didn't realize the pill fell out of my bag."
Plan of Correction: Staff changed the child's shirt, rinsed her mouth, cleaned her face/hands, and monitored for symptoms until first responders arrived. The tAssistant Director, contacted 911, poison control, and the parents who arrived a few minutes later.

Medication shall be kept in a locked place using a safe locking method that prevents access by children. One staff medication fell out of an unlocked bag and was accessible to children. A preschool child found the pill and broke open the capsule.
Plan of Correction: All staff will lock up any medications that are brought into the center.
