Inspector Notes
An unannounced monitoring inspection was conducted on August 6, 2025 in response to a self-report received by the licensing office on July 29, 2025 relating to supervision. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff. On July 22, 2025, a 3 year old child was left in the bathroom unsupervised for 7 - 10 minutes. The child was discovered by a representative of another child care. Staff were not aware that the child was not with the class.
Plan of Correction: Staff were reprimanded for lack of supervision. Parents were notified.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards. On July 22, 2025, Staff 1, Staff 2, and Staff 3 did not follow center supervision policies which requires that "teachers MUST do head counts of the children in their care frequently. If two or more teachers are working together, they need to communicate regularly regarding the number of children in their care". As a result of the policy not being followed , Child #1 (3 years old) was left unsupervised in the bathroom for approximately 7 to 10 minutes.
Plan of Correction: Staff were reminded of sight and sound supervision policies.
