Inspector Notes

A monitoring inspection was initiated on 10/4/2023 and concluded on 10/4/2023 from 3:35 PM to 4:35 PM. There were 18 children present, ranging in ages from 10 years to 12 years, with 1 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the three staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation and interview, the center failed to ensure a qualified program leader or program director and an immediately available staff member are present at all times.

Evidence:

1. Only one staff member was present when the licensing inspector arrived until 4:25 PM when a second staff member arrived.
2. The staff member that was present at the time of arrival is the program director and he verified he was the only staff member present that day due to staff calling in sick.
Plan of Correction: Will ensure there will be adequate staff present at start of the program. Will ensure staff call out in advance so substitution staff can be called in.
