During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 9, 2024 and completed on December 13, 2024. The on-site inspection began at 3:10 p.m. and ended at 4:11 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and two staff. The inspector reviewed five children?s records and two staff records electronically on December 12, 2024. This inspection included documentation review, tour of the facility, observation, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The medication authorization shall be available to staff during the entire time it is effective.
One long-term medication did not have a parent authorization and one long-term medication did not have a parent or physician authorization.
Plan of Correction: The parent of the child with the two medications will be informed to provide the authorization forms.
