Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on August 6, 2021 and concluded on September 2, 2021. A self-reported incident was received by the department regarding allegations in the areas of forbidden actions. The director was contacted by telephone and a virtual inspection was conducted. The inspector emailed the director a list of documentation required to complete the investigation and interviews were conducted.

The evidence gathered during the inspection did not support the self-report of non-compliance with standards or law. No violations were issued.
