Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on August 6, 2021 and concluded on September 2, 2021. The department received a self-reported incident regarding allegations in the area of forbidden actions. The director was contacted by telephone and a virtual inspection was conducted. The inspector emailed the director a list of documentation required to complete the investigation and conducted interviews.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on interviews and review of documentation, it was determined that on July 30, 2021, a staff engaged in a forbidden action by rough handling a child.
Evidence: 1. Based on an interview with an outside witness, the witness observed Staff #1 grab the arm of Child #1 "a little hard" and pulled the child up and placed her in a corner to sit in a chair. The witness stated a red mark was left on Child #1's arm and provided a photo of the red mark.
2. The photo of the red mark was observed. There was a red mark on the upper part of the child's left arm.
3. Administration reported Staff #1 grabbed Child #1's arm and left a red mark on the child's arm. Administration reported Staff #1 was terminated stating Staff #1's actions "did not exemplify Goddard and State Standards."
Plan of Correction: Staff #1 was immediately terminated. These actions do not represent our standards. The rest of the faculty was also retrained on proper handling and our expectations as a school.
