Inspector Notes
The licensing inspector conducted an inspection 03/07/2023 regarding an incident the center reported to licensing regarding a child being left on the playground on 01/23/2023 unsupervised. Staff interviews, observations, and review of documents were conducted.
The preponderance of the evidence gathered during the inspection supported a violation of the Standards for Licensed Child Day Centers.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Violations
1Evidence:
According to reports and interviews, child, C1, age 4, was left alone on the playground on 01/23/2023 for approximately 3.5 minutes and the classroom teacher, S1 did not know the child was left alone on the playground until the child?s parent came to pick him up for the day. The approximate temperature at 4:30 p.m. on 01/23/2023 was 50 degrees Fahrenheit. The incident report prepared by the center documented that S1 did not do the ?name-to-face threshold check when the children entered the classroom and that C1 was outside the classroom door alone from 4:28:27 to 4:31:57.?
