Inspector Notes

An unannounced inspection was initiated on 04/04/2024 and completed on 04/12/2024 with local agencies, in response to a complaint received relating to allegations of programs. The inspector reviewed 3 child records and 7 staff records on-site. This inspection included document, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on

The following action is forbidden: forcing rest. Interviews were conducted on 04/04/2024 and 04/12/2024 by licensing and other agencies regarding the care of children in the classroom by S1. Staff interviewed reported observing S1 holding C1 on her cot and putting a blanket over her body and head during naptime and holding it with her arm. She acknowledged she has put a blanket over C1?s head while the child was on her cot at nap time.
Plan of Correction: Employee has been terminated All staff were retrained on proper naptime procedures.
